Patient Care Coordinator

UW Medicine Montlake- Surgical Specialties/ Thoracic Surgery Specialties has an outstanding opportunity for a Patient Care Coordinator.

Work Schedule:

• Full-time, 100% FTE (40 hours per week)
• Day Shift
• Mondays- Fridays/ 8:00am- 4:30pm

Primary Job Responsibilities:

• Uses ability to triage medical information in the Thoracic Surgery department within Surgical Specialties to a variety of Health Care Professionals using effective oral and written communication
• Works independently under the general direction of a physician or their designee
• Coordinate pre- and post-surgery diagnostic procedures, tests, and follow-up patient appointments
• Act as a liaison between patients, consulting physician, referring physician, or outside agency to coordinate patient appointments, admission, anesthesia, and surgery.
• Obtain all necessary medical records, diagnostic studies, pathology items, etc., that are needed for patient care
• Demonstrates the ability to be organized and effectively prioritizes work tasks for the needs of the clinic.
• Requests interpreters as appropriate for appointments, admissions, and surgery.
• Collaborate with all members of the clinic team to ensure exceptional patient care and satisfaction.
• Perform other duties as assigned

Requirements:

• A Bachelor's degree in a medically related or public relations field, AND two (2) years experience in a tertiary patient care setting involving interaction with a variety of medical services, contact with patients and families, interaction with physicians, and proficiency in medical terminology; OR Equivalent education/experience.
• Equivalent education and/or experience may substitute for minimum qualifications except when there are legal requirements, such as a license, certification
