Job description:
- Setting: Skilled Nursing Facility, Sub-acute and long-term care patients
- Schedule: Minimum of (2) days on site per week. Telehealth support is available on-site for virtual interactions
- Must be licensed in VT
- Experienced in SNF preferred, not required
