Schedule: Exempt, minimum 40 hours per week | 7 days on/7 days off on call rotation with weekend hours Department: Participant Care | Huron Valley PACE | Ypsilanti, MI What You'll Do In This Role: Start your nursing career at Huron Valley PACE! As an Extended Care Nurse at Huron Valley PACE, you’ll provide essential after-hours and weekend care, ensuring participants receive timely, compassionate support when they need it most. Working a unique 7-on-7-off schedule, you’ll take nursing and intake/referral calls, assess patient needs, and coordinate both scheduled and as-needed visits - helping manage complex conditions and prevent unnecessary hospitalizations.
