BrightCare HomeCare, a leading provider of home care services in Louisiana, is seeking a dedicated and compassionate Care Coordinator - Home Care to join our team! Responsibilities
Coordinate and manage the care plan for clients, ensuring their unique personal and health needs are met
Communicate effectively with clients, families, and healthcare professionals to ensure seamless transitions and optimal care delivery
Develop and maintain positive relationships with clients, families, and caregivers to promote a sense of trust and respect
Monitor and evaluate client progress, making adjustments to the care plan as necessary
Collaborate with our multidisciplinary team, including nurses, therapists, and caregivers, to ensure comprehensive care delivery
Maintain accurate and detailed records of client interactions, care plans, and client outcomes
Participate in ongoing training and education to stay current with industry best practices and regulations
