Key Responsibilities\n

• Collaborate effectively with a multidisciplinary team through consistent communication, documentation, and participation in meetings, including patient rounds, to ensure a coordinated approach to client care.

• Provide case management services, which may involve consultation with hospital and community resources, aiding in the continuity of care and access to necessary support services for clients.

• Document clinical interactions and treatment plans promptly and ethically, adhering to departmental standards to ensure the delivery of quality care.

• Utilize evidence-based therapeutic modalities, such as Cognitive Behavioral Therapy (CBT), in addition to other methods that align with clients' personalized treatment goals.
