Trinity Health is seeking a Weekend Track RN for the Community Hospice Admissions Team to admit patients to hospice services in home settings, with cross-training across levels of hospice care. The role focuses on weekend coverage, two home admissions daily, and close collaboration with physicians and an interdisciplinary team.
