Physical Therapist - Home Health Assignment Duration This is a 13-week contract position. Schedule Day shift, Monday through Friday, 8:00 AM to 5:00 PM. Job Description We are seeking a skilled Physical Therapist to join our home health team. The ideal candidate will have experience in adult healthcare and be capable of managing 6 to 7 visits per day. This role involves conducting evaluations and developing treatment plans for patients in the home care setting. Responsibilities
- Conduct patient evaluations and create personalized treatment plans.
- Deliver therapy interventions within the home health setting.
- Monitor patient progress and adapt treatment plans as necessary.
- Collaborate with interdisciplinary teams for comprehensive patient care.
- Minimum of 2 years of physical therapy experience preferred.
- Current MD PT State license required upon submission.
- BLS certification required.
- Previous charge experience preferred.
