Job Summary

Cuready Healthcare Staffing is seeking a qualified RN, Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

• Coordinate care for patients with heart transplants, including immunosuppression and routine patient monitoring.


• Manage prophylactic antibiotic administration and monitor for organ rejection and infection.


• Collaborate with attending physicians and multidisciplinary teams to adjust medication and care plans as needed.


• Facilitate communication between patients, families, and healthcare professionals regarding care issues.


• Support continuity of care across the transplant process as per provider orders and protocols.


• Assess educational needs of patients and families, coordinating with financial and social services as necessary.

Qualifications

• Current California RN License and AHA BLS certification.


• 1 year of experience in ambulatory care, with proficiency in cardiac nursing and management of heart transplant recipients.


• Experience floating to other facilities and compliance with UCSF guidelines.


• Proficiency in Epic charting system and skills in care coordination and disease management.


• Preferred experience in managing other organ transplant recipients and knowledge of immunosuppressive therapies.


• COVID-19 and flu vaccines required, with booster unless medically exempt.
