The role requires quick adaptation, efficient work within a team environment, and collaboration with a team of social workers. Minimal training is provided, thus experience in similar roles is critical for this position. Responsibilities:
- Provide acute inpatient case management and discharge planning.
- Collaborate with social work team to manage caseloads of 15-20 patients per day.
- Consult with respiratory therapy and pharmacy for discharge planning as needed.
- Cover all inpatient units throughout the hospital, coordinating complex placements.
- Minimum 2 years of acute inpatient medical case management experience, including 1 year in a large, busy hospital ED.
- Recent acute care discharge planning experience required.
- Proficiency in EPIC charting and strong computer skills.
- BLS Certification.
- Previous travel experience in a level 1 facility within the last 3 years.
- Experience outside the required scope, such as telephonic or insurance review experience, is not suitable for this role.
