RN (Registered Nurse), Case ManagerCity/State: Jackson, MississippiShift: 8H Days 8 hr days, 08:00:00-16:00:00Contract Length: 13 weeksStart Date: 08/06/2026Job Description:The Case Manager is responsible for coordinating patient care and discharge planning for a variety of patients within a Level 1 trauma hospital. The role requires experienced case management with a focus on acute inpatient care and minimal training provided on the unit.Responsibilities:Coordinate care and discharge planning for 15-20 patients per day across all inpatient units.Work collaboratively with a team of 15 full-time and 17 part-time social workers.Consult with Respiratory Therapy and Pharmacy as needed for discharge planning.Utilize EPIC for charting and manage patient documentation efficiently.Requirements:Minimum 2 years of Acute Inpatient Medical Case Management experience, including 1 year in a large busy hospital ED (500+ beds, LEVEL 1/ACADEMIC Medical Center).Recent acute care discharge planning experience for patients with diverse medical diagnoses is required.Proficient in EPIC charting and possess strong computer skills.BLS Certification required.Previous travel experience in a Level 1 facility within the last 3 years.Client Details City Jackson State MS Zip Code 39216
