Job Description:

We are seeking a highly experienced RN Case Manager with acute inpatient case management and discharge planning expertise. The role requires handling complex patient cases across all inpatient units, working collaboratively with a large social work team. The candidate must be proficient in EPIC charting and have recent acute care discharge planning experience.

Responsibilities:

Manage 15-20 cases per day across various inpatient units.
Coordinate with the social work team and consult with RT and Pharmacy for discharge planning needs.
Ensure compliance with financial assistance criteria for uninsured or underinsured patients.
Participate in a department-specific orientation and work alongside the case management team.

Requirements:

Minimum 2 years of acute inpatient medical case management experience, including 1 year in a large busy hospital ED (500+ beds, Level 1/Academic Medical Center).
Recent acute care discharge planning experience with diverse medical diagnoses.
Proficiency in EPIC charting and strong computer skills.
Previous travel experience in a Level 1 facility within the last 3 years.
BLS Certification required.
