Capital Health is seeking a Registered Nurse to provide clinical management and diabetes self-management education to patients, in collaboration with physicians and the diabetes care team. The role involves planning educational programs, developing materials, and involving patients and families in teaching and learning processes to achieve optimal outcomes.
The educator will formulate individualized care plans, integrate cultural considerations, and work with dietitians to tailor nutrition
