Job Summary:
The organization is seeking a Physical Therapist (PT) for outpatient services at an acute care hospital located in Santa Rosa, California. The candidate will be responsible for providing outpatient therapy to adult and geriatric patients, focusing on rehabilitation and recovery. Candidates are required to bring their valid California license on the first day of the assignment.

Job Details:

• Location: Santa Rosa, California
• Position: Physical Therapist (Outpatient) PT OP
• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week from June 15, 2026
• Required Qualifications: Valid California license; BLS certification; 0 years of experience required
• Preferred Qualifications: Acute care outpatient rehabilitation experience; willingness to float to acute rehab units with proper orientation; experience with Epic charting system; minimum charge experience of 1 year; references covering 1 year (6 months for CNAs) from the last 3 years; ability to work 2 out of 3 major holidays (Thanksgiving, Christmas, New Year); ability to float between sister facilities if needed
• Additional Information: Travelers may be expected to float between sister facilities nearby the contracted hospital. Refusal to float may result in termination of assignment and forfeiture of guaranteed hours.
• Certifications Needed: BLS; specific Epic charting experience required
• References: Two references covering the past year (or six months for CNAs) during compliance and prior to start
• Vaccinations: COVID-19 vaccine not required; flu vaccine status unknown
