Care Manager-RN

$35.92 - $53.88 per hour

Care Manager-RN Pay Range: $35.92 - $53.88 per hour. Actual pay will be based on experience, skills, training, certifications and education. Hospital Sisters Health System (HSHS) invites a qualified RN to join our NICU team. The Care Manager collaborates with patients, families, social workers, physicians, the interdisciplinary team and payer representatives to facilitate efficient and cost‑effective progression through the continuum of care. Responsibilities Ensure patient progression through the continuum of care in an efficient and cost‑effective manner. Identify, initiate and manage optimal care coordination and discharge planning to improve patient flow, throughput, continuity of care, and patient satisfaction. Conduct comprehensive assessment, planning, implementation and evaluation of individual patient needs. Monitor provision of cost‑effective care to ensure care is delivered in the most clinically appropriate setting. Facilitate compliance with regulatory requirements and organizational performance standards, including core competency demonstration. Collaborate with the interdisciplinary team to ensure seamless transitions and high quality of care. Location & Schedule Department: Care Management Facility: St. John’s Hospital, Springfield, IL Schedule: Full Time, 40 hours/week with weekend rotation Education & Experience Associate’s Degree or Diploma in Nursing required. Bachelor’s Degree in Nursing preferred. Minimum 2 years of clinical experience required. Prior case management experience preferred. Quality/Process Improvement experience preferred. Certifications & Licenses Registered Nurse (RN) license in state of practice required. In Wisconsin, a valid multi‑state RN license may be used under the Nurse Licensure Compact. Basic Life Support (BLS) certification required within 60 days of hire. Case Management certification (ACM, CCM, RN‑BC) preferred.
