The work schedule is Monday to Friday, 9 am to 5 pm with one weekend per month. RNs will work 5 days a week, 8‑hour shifts (options: 8 am–4 pm, 9 am–5 pm or 10 am–6 pm). RNs should be prepared to work within the hospice referral sources in both hospital and outpatient settings (MD offices, clinics, SNFs, etc.). Serve as a key intermediary between the hospice organization, hospital staff, patients, and their families to ensure clear and effective communication and coordination of hospice care, including business development. Conduct thorough chart assessments of patient cases and bedside evaluations to determine the clinical picture and hospice eligibility. Collaborate with facility clinical team and hospice physicians to make informed decisions regarding patient care. Successfully facilitate numerous hospice admissions, ensuring seamless transitions for patients and their families. Provide critical support and education to enhance patient and family understanding of hospice care, leading to improved satisfaction and care outcomes. Oversee the coordination of admission assessments and procurement of durable medical equipment, disposition planning, ensuring that all patient needs are met. Jobot is an Equal Opportunity Employer.
