Part-time Physician – Post-Acute Care | Flexible Schedule | Wharton, TX Exciting Opportunity in a Growing Physician-Led Organization Trumen Physicians + Associates is seeking a dedicated and experienced Physician to join our growing post-acute care team in Wharton, TX. You’ll play a key role in reducing hospital readmissions, managing chronic and acute conditions, and guiding patients through transitional care with compassion and continuity. Core Responsibilities: * Provide in-person medical care to patients in assigned post-acute facilities * Perform assessments, manage treatment plans, and update documentation in the EMR * Lead or collaborate with facility teams to ensure quality outcomes and smooth transitions of care * Participate in Trumen’s value-based care and transitional care programs * Develop strong working relationships with facility staff and leadership * Serve as a liaison between Trumen and facility teams * Be available in assigned facilities anytime during regular business hours on scheduled days Qualifications: * MD or DO with a current, unrestricted Texas medical license * Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine * 1+ year of experience in post-acute, geriatric, or SNF care preferred * Comfortable practicing independently in skilled nursing facilities * Strong clinical judgment, communication skills, and a patient-centered mindset * Reliable transportation within the Southwest Houston area About Trumen Physicians + Associates: Founded in 2010, Trumen is a physician-led organization committed to improving outcomes in post-acute and transitional care settings.
