Licensed Practical Nurse (On Call/PRN)

Responsibilities:

• Provide direct patient care to residents, including administration of medications, wound care, and vital sign monitoring

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Perform routine tasks such as bathing, dressing, and grooming residents

• Observe and report any changes in residents' conditions to the registered nurse or other healthcare professionals

• Maintain accurate and detailed records of patient care and treatment plans

• Participate in ongoing education and training to stay current with best practices and industry developments

Requirements:

• Current LPN licensure in the State of Minnesota

• High school diploma or equivalent required; associate's degree or higher preferred

• Ability to lift, transfer, and reposition patients as needed

• Strong communication and interpersonal skills

• Ability to work effectively in a fast-paced environment
