Home Health Field Registered Nurse (RN)

Key Responsibilities Provide hands‑on patient care within RN scope (e.g., PICC lines, wound care, feeding pumps, NG/OG/J tube management, suture removal, various drains, Foley catheter care, pleural drainage, tracheostomy care, blood draws, ostomy management, injectable therapies, IV antibiotics/TPN, compression wraps). Execute direct patient care as defined by the state Nurse Practice Act. Implement a plan of care initiated and directed by the Case Management Registered Nurse. Document accurately and timely within 24‑48 hours, consistent with the plan of care. Assess and educate patients, families, and caregivers on diagnoses and care plans. Assist the registered nurse with skilled procedures and prepare equipment for treatments. Coordinate home health services, reporting interdisciplinary needs to the Case Management RN. Use equipment and supplies efficiently. Participate in personal and professional growth and development activities. Carry out additional duties assigned by the Case Management RN. Retrieve and prioritize patient communication, giving timely reports to the Case Manager. Record concise, relevant information reflecting patient needs and responses to interventions. Accurately record any incidents affecting the patient. Report pertinent information promptly to the Case Management RN and to providers. Attend weekly Case Conferences for coordination and communication. Maintain patient privacy in records and care. Maintain professionalism and pursue opportunities to enhance clinical knowledge. Meet productivity standards as directed by the Case Management RN. Review and complete Start of Care assessments and plans of care with the Case Manager. Participate in on‑call duties per weekend rotation. Qualifications One year of recent nursing experience (waivable for internal candidates or new graduates). Preferred: One year of home health care experience. Experience with high‑acuity patients is a plus. Experience working with interdisciplinary teams is highly desired. Bachelor's degree from an accredited nursing school. Current California Registered Nurse (RN) license. Current Basic Life Support (BLS) card. Valid California Driver's license with acceptable driving record. All licenses and certifications must be current at hire and sustained throughout employment. Self‑directed, critical thinker, outstanding interpersonal skills, organized, computer literate, excellent observation, verbal and written communication, problem‑solving, basic math skills; nursing skills per competency checklist.
