Geriatric Care Manager

American Academy Of Family Physicians

The American Academy of Family Physicians in Atlanta is seeking a licensed physician to manage care for members in a Skilled Nursing Facility (SNF). You will be part of a collaborative team that addresses medical needs, care planning, and transitions of care while documenting effectively.
