Responsible for developing, coordinating, and overseeing all aspects of the Memory Care Program to ensure residents living with Alzheimer’s disease, dementia, and other cognitive impairments receive compassionate, person-centered care. This role creates and maintains a therapeutic environment that promotes each resident’s highest level of well-being, dignity, independence, and quality of life while supporting residents, families, and team members.

RESPONSIBILITIES
ESSENTIAL FUNCTIONS
Directs and supervises Memory Care Staff.
Develop, maintain and periodically update written care policies, procedures and manual, objectives and philosophies. Assist in developing and implementing methods for coordinating care services with other resident services.
Implement plans of correction for state surveys and deficiencies, complete medical forms, reports, evaluations, studies, charts, etc. Implement and maintain the continuous quality improvement program.
Schedule department work hours, including vacation and holiday schedules, shift and work assignments. Ensure adequate staffing and expedite work; delegate administrative authority, responsibility and accountability to resident care services supervisory associates as deemed necessary to perform assigned duties.
Coordinates the implementation of the Memory Care Program mission, goals and philosophy.
Provides or secures training to associates in the Community including training caring for residents with memory impairment and other advanced training.
Provides or secures the additional training/practicum experience to Resident Caregiver associates of Memory Care Program.
Coordinates with Resident Care Director in establishing and maintaining a staffing pattern to meet our customized care commitment.
Maintains a mentally, physically and spiritually stimulating activity program that is geared to the individual’s preferences and capabilities.
Participates with the Resident Care Director in the hiring, scheduling and supervising of Resident Care Associates assigned to the Memory Care program.
Complete yearly training materials and any other training deemed necessary by management.
Acts as liaison to facilitate communications among Memory Care, the community, family members and the local community.
Maintains an environment in the residents’ personal and congregate areas, which will help to compensate for cognitive, sensory and physical impairments.
Interviews residents and families prior to admission. Coordinates the pre-admission screening and assessment process with Directors of Marketing and Resident Care.
Coordinates with the Resident Care Director, Memory Care Program Caregivers and families in the development of the 24-hour plan for the residents and updates the plan in a timely manner.
Ensure services are delivered consistently according to individualized care plans.
Attends Department head meetings and leads regularly scheduled Memory Care Program meetings.
Provides Caregivers with updates and in-service training programs and Memory Care programs.
Employees are required to notify the Company immediately if their license or certificate expires, lapses, is suspended, restricted, revoked, or otherwise becomes inactive.
Licensure Requirement: Employee must maintain a current, valid Georgia CNA certification or Georgia LPN license,Georgia RN license (as applicable) in good standing throughout employment. Failure to maintain the required license or certification may result in removal from duties requiring licensure and may affect continued eligibility for the position, in accordance with applicable law.
Assists the Marketing Director in promoting the Memory Care Program to doctors, hospital discharge planners and local community agencies.
Maintains recruiting of all Memory Care positions, conducting interviews, hires, onboarding, training, motivation, and retention.
Coordinates departmental schedule to the director of resident care.
Participate as instructed in the sales process of potential residents.
Coordinates the Memory Care Program volunteer/mentor program.
Develops and maintains the community’s resource library.
Models our company philosophy, helping out whenever needed to meet residents’ needs and working as a member of the overall team.
Participate as a Manager on Duty during assigned weekends.
Community Leadership & Emergency Response: Expected to serve as essential leadership personnel during inclement weather and emergency protocols. This includes on-site presence or active coordination of community safety,staffing levels, and resident care in accordance with state regulations.
Conduct yourself professionally and contribute to, service-oriented environment through their appearance, communication, and interactions with residents, families, visitors, and colleagues.
Any other duties as assigned by the Executive Director and management.
QUALIFICATIONS
Must be 21 years of age, or older
Must have satisfactory criminal background check in accordance with state requirements.
Must read, write, speak, and understand English.
Must demonstrate patience, tact, enthusiasm, and positive attitude towards the elderly.
EDUCATION AND EXPERIENCE
Previous successful experience in a memory care unit is preferred.
Registered Nurse or Licensed Nurse in good standing who meets all federal and state licensure requirements, or an individual holding current med tech certification or certified dementia practitioner training.
Prior experience working with older adults with memory impairment.
Two years of experience in geriatric care.
Demonstrated supervisory experience.
PHYSICAL REQUIREMENTS
Must have upper body strength adequate to bend, lift, shift, move, twist, and/or assist in moving articles of more than twenty-five pounds.
Must be able to squat, reach, and stretch without distress.
Must be able to tolerate extended periods of walking and standing.
