Department Overview Family MedicineatEast Portlandis aFederal Qualified Health Center(FHQC)providingprimary care.The patient population includes pregnant people, babies, children,adolescents, adults, andgeriatrics. The patients aregenerally moderatelystable to moderately unstable, with complex medical conditions and SDOH barriers that require medical and nursing interventions. Responsibilities include remote phone triage, MyChart message management,resultsreview and follow-up, transitions of care, and other duties aligned with operational needs to support clinic workflows and the RN team. ProviderServices Offered: Primary Care Behavioral health and counseling services Reproductive health services Transgender health care Prenatal care (including deliveries) Project Nurture Program Pediatric care Medication-assisted treatment (MAT) Minor procedures Health maintenance with preventive care Most nursing work occurs outside of scheduled visits and services include Nurse telephone triage, with in-clinic Nurse triage OB intake and support visits Wound Care Medication Administration Patient education Transitions of Care and Care Coordination Team based care and interdisciplinary collaboration Chronic disease management Foot care clinics Required Qualifications BSN Graduates: Baccalaureate Degree in Nursing from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA) 30 days before start date. ADN Graduates: Associate Degree in Nursing from an accredited program 30 days before the start date. Associate degree Nurses required to enroll in BSN program within 3 years of hire and complete within 5 years of hire Current, unencumbered Oregon State Registered Nurse License Washington RN License within 6 months of hire BLS from AHA required. New hires will be enrolled and required to complete during orientation. Must be able to perform the essential functions of the position with or without accommodation Preferred Qualifications Experience in independent RN triage via phone and in-basket messagingutilizingprotocols Conducts RN led visitswithin scopei.e.iv placements, medication administration, chronic disease education,wound care,etc Experience in longitudinal care managementand care coordination Current Basic Life Support (BLS) issued by American Heart Association (AHA) Additional Details Length of Orientation – New Graduate Nurse External candidates: OHSU & Nursing New Employee Orientation (NEO) for about a week. Either Transition to Practice (TTP) Program Residency Learning Pathway or Orientation experience for about 10-26 weeks depending on the care area. An employment service agreement may apply.
