Responsibilities:

• Provide physical therapy services to patients in their homes and communities, including evaluation, treatment, and discharge planning

• Develop and implement individualized treatment plans to promote patient recovery and independence

• Collaborate with healthcare team members to ensure comprehensive care and optimal patient outcomes

• Participate in interdisciplinary team meetings and contribute to the development of patient care plans

• Conduct home visits and communicate effectively with patients, caregivers, and family members

• Maintain accurate and detailed records of patient care and progress

Requirements:

• Current New Jersey Physical Therapist license

• Doctorate in Physical Therapy (DPT) or related field

• Strong clinical skills and knowledge of physical therapy techniques and best practices

• Ability to work independently and as part of a team

• Excellent communication and interpersonal skills

• Strong organizational and time management skills

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $84,000 to USD $112,000 When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions.
