Job Overview As a Nurse Practitioner at Klamath Health Partnership, Inc., a Federally Qualified Health Center (FQHC), you will be at the forefront of providing high-quality, patient-centered care to a diverse community. You'll assess, diagnose, and manage patient health conditions while promoting preventive health measures. This role emphasizes compassionate care for underserved populations, focusing on improving health outcomes and addressing social determinants of health. You will work collaboratively within a multidisciplinary team, providing holistic care and advocating for patients in an inclusive and culturally sensitive environment. Key Responsibilities Clinical Care Perform comprehensive patient evaluations, including health history, physical exams, and necessary diagnostic tests. Develop and implement individualized treatment plans, focusing on preventive care and chronic disease management. Provide primary care services such as health education, immunizations, minor procedures, and screenings. Counsel and educate patients and their families on health conditions, preventive measures, and lifestyle adjustments to support long-term wellness. Collaborate with physicians, nurses, behavioral health providers, and other team members to ensure coordinated, comprehensive patient care. Compliance and Documentation Maintain accurate, thorough, and timely documentation in alignment with FQHC protocols, HIPAA, and regulatory requirements. Participate in quality improvement initiatives and follow clinic guidelines to support high standards of care. Conduct follow-up appointments and ongoing evaluations to monitor patient progress and adjust care plans as needed. Community and Patient Engagement Serve as a patient advocate, connecting patients with resources and support services within and outside the clinic. Participate in community outreach and health education events to promote preventive care and raise clinic awareness. Engage patients with empathy and cultural sensitivity, recognizing and addressing social factors that impact health. Qualifications Education : Master's or Doctorate degree from an accredited Nurse Practitioner program. Certification : Current certification as a Nurse Practitioner (FNP, PNP, or relevant specialty) and state licensure. Experience : Previous experience in a community health or FQHC setting is preferred but not required. Skills : Strong diagnostic skills, excellent communication and interpersonal abilities, and a collaborative approach to patient care. Other : Bilingual or multilingual skills are a plus, reflecting the needs of the patient population served. Why Join Us?
