This role involves providing comprehensive nursing care in various community settings, ensuring health maintenance and disease prevention, and managing case-related activities. The Registered Nurse will work closely with a multidisciplinary team to implement and evaluate health programs.

Key Responsibilities of the RN - Home Health

• Observe, assess, plan, implement, and evaluate nursing support for individuals in community settings
• Collaborate with health professionals to address the health needs of persons served
• Coordinate communications and actions regarding health and safety supports
• Manage medications according to physician prescriptions and ensure policy compliance
• Document health status and report changes to the physician as needed
• Maintain equipment and supplies to prevent disease spread
• Supervise licensed practical/vocational nurses and unlicensed staff
• Provide training and recertification in health and safety topics

Requirements of the RN - Home Health

• WV RN License
• Experience in Home Health
• Proficiency in HCHB and OASIS
