The RN collaborates with an interdisciplinary team to develop individualized care plans and deliver skilled nursing services in the patient’s home.This position is ideal for nurses who value autonomy, patient interaction, and the ability to make a direct impact.ResponsibilitiesDevelop, implement, and revise individualized plans of care with patients, caregivers, and providersProvide skilled nursing care, including preventive and rehabilitative services per physician ordersDeliver patient and caregiver education to promote independence and safetyOffer counseling and support to patients and familiesMaintain accurate and timely clinical notes and documentationCommunicate regularly with physicians and healthcare team members regarding patient statusIdentify and coordinate discharge planning needsEnsure care is delivered in compliance with all regulatory and company standardsQualificationsActive California RN license (required)At least 2 years of nursing experience, preferably in home health or public healthCurrent CPR certificationValid driver’s license and insured, reliable vehicleStrong clinical judgment, communication, and documentation skillsAbility to work independently and manage time effectivelyAble to lift, transfer, and position patients as neededComfortable with walking, standing, bending, and other physical activity required in home careAbout Buena Vista Home HealthBuena Vista Home Health is part of The Pennant Group, a nationwide network of over 300+ home health, hospice, and senior living locations.
