We're a place to belong. As a Hospice Nurse Case Manager (RN) , you will coordinate and deliver individualized, patient-centered care in patients' homes and care facilities. You'll build meaningful relationships with patients and families while working both independently and collaboratively with an experienced interdisciplinary team. In this role, you will help ensure comfort, dignity, and quality of life while guiding patients and their loved ones through the caregiving journey with compassion, confidence, and clinical expertise. Key Responsibilities include: Develop, implement, and oversee individualized plans of care based on each patient's goals, needs, and preferences. Deliver comprehensive nursing care in home and facility settings. Coordinate medical, emotional, psychosocial, and spiritual support services with the interdisciplinary team. Perform ongoing assessments and adjust care plans as patient needs evolve. Administer medications and manage complex symptoms to promote comfort and quality of life. Educate patients and families regarding disease progression, symptom management, and caregiving strategies. Serve as a trusted advocate and resource for patients and families navigating end-of-life decisions. Collaborate closely with physicians, advanced practice providers, and interdisciplinary team members. Provide emotional support and compassionate guidance throughout the hospice journey. Maintain accurate and timely clinical documentation in the electronic medical record. Participate in interdisciplinary team meetings and quality improvement initiatives. REQUIREMENTS / QUALIFICATIONS:

A graduate of an approved school of nursing (RN/BSN/ARNP) with a current Iowa Nursing License. Two years of home health or hospice or work related experience preferred. General medical knowledge related to home health and hospice services with end-of-life issues is desirable. Car, driver's license and adequate insurance as outlined in the personnel policies is required. Strong clinical knowledge, critical thinking skills, and evidence-based practice expertise. Excellent communication, interpersonal, and organizational skills. Ability to prioritize tasks, delegate responsibilities, and manage multiple priorities effectively. Proficient computer skills, including electronic health records (EHR) and Microsoft Office applications. Advanced Certifications in Hospice and Palliative Nurse certification by the 3rd year of employment. KNOWLEDGE/SKILLS:

Overall, the role requires strong interpersonal skills, negotiation abilities, and a deep understanding of healthcare operations, regulations and cultural differences. Demonstrates competence in professional decision-making skills within the parameters of the Nurse Practice Act, including the ability to make appropriate assessments, problem-solve, prioritize, complete Plans of Care and evaluate their effectiveness. Demonstrates initiative in recognizing unmet needs and assuring a timely and appropriate response, skills and knowledge in providing education and treatment of acute and chronic symptom management. Maintains confidentiality in all matters, including employee information, client information and agency matters. Knowledge of computer operating systems, data sourcing, project management with outcomes analysis proficiency required. Demonstrates organizational skills necessary to manage workload and productivity expectations while working independently and managing several projects simultaneously. Exhibits professionalism in all workday interactions, follows through on commitments and maintains composure under stress. Recognizing new referral opportunities and ensuring that the organization responds appropriately to them.

WORK ENVIRONMENT/EQUIPMENT:

Perform the duties in both inside and outside environmental conditions. Visits are conducted in a variety of settings including family home, group home, LTC and assisted care facilities and requires employee to drive from location to location during the work period in good and inclement weather, within the service territory including city and small towns and rural areas. Atmospheric conditions – fumes, odors, dust, poor ventilation, animal presence, and/or smoking. The ambient noise level in this environment can flex, is generally low. Duties include frequent use of electronic devices: iPad, I Phone, desktop computer, copier and occasionally other office machinery. The driving requirements include in-town and highway driving in good and inclement weather.
