In-Home Nurse Practitioner Part Time - Saint Louis, MO

Job Summary Signify Health, part of CVS Health, seeks a Part‑time Nurse Practitioner to deliver in‑home health evaluations in members’ homes. The role focuses on patient‑centered care, collaborative relationships, and bridging gaps in health delivery across a national network. Responsibilities Conduct in‑home health evaluations lasting approximately 45 minutes, including medication and medical history review, physical examination, and diagnostic procedures as ordered by the member’s health plan. Use an iPad and an intuitive clinical workflow to deliver each evaluation; not responsible for prescribing medication, ordering lab tests, or altering current treatment regimens. Build trusted relationships with patients, deliver personalized care plans, and connect members to appropriate providers or services. Work collaboratively with Signify Health clinician colleagues and fulfill the requirements of the specified role. Travel locally and within licensed states as needed; conduct virtual visits when required. Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, diabetic eye exams, DEE). Qualifications Active, unrestricted license(s) in coverage area(s). Board certification required. Multi‑state licenses preferred. Compensation & Schedule Anticipated weekly hours: 29.5. Part‑time. Pay range: $37.66–$81.11 per hour (base rate); actual base salary will depend on experience, education, geography and other factors.
