LTC/SNF LPN

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Long-Term Care/Skilled Nursing Facility (LTC/SNF) team. The ideal candidate will demonstrate clinical expertise, along with a strong commitment to providing exceptional care to residents.

Shift Information

Day and Evening Shifts available
Shift Length: 8 hours
Schedule includes every other weekend and holiday

Responsibilities

Provide direct patient care and assist in the development of nursing care plans.
Administer medications and treatments as prescribed by the healthcare provider.
Monitor and document patient progress and any changes in condition.
Communicate with patients and their families about health status and care plans.
Participate in team meetings and contribute to care planning.

Qualifications

Must have at least 2 years of experience in a nursing role within a LTC/SNF environment.
Current BLS Certification required.
Strong interpersonal and communication skills.
Ability to work effectively as part of a team and independently.

This position guarantees 40 hours of work per week, providing a consistent schedule for a work-life balance.
