Primary Care Physician physician employment in Florida : Become a part of our caring community Conviva Senior Primary Care, a subsidiary of Humana Inc., is recruiting a full-time primary care physician for our Normandy center in Jacksonville, Florida. We have a unique, value-based care model and we are experts in senior-focused primary care. As a Primary Care Physician, you will report to our Chief Medical Officer/Regional Medical Director. The Physician focuses on outpatient medicine, continuity of care, health maintenance, and disease prevention. Keeps a medical history and medical records. Refers the patient to specialists as needed. Begins to influence department's strategy. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $255,000 - $298,500 per year This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
