The Licensed Practical Nurse (LPN) is responsible for providing direct patient care under the supervision of a registered nurse and in concert with our values: trustworthy, capable, compassionate, proactive, and called. Essential Functions Provide direct patient care in accordance with the state Nurse Practice Act. Implement plan of care initiated by the registered nurse. Assess and provide patient and family/caregiver education regarding self‑care techniques and information pertinent to diagnosis and plan of care. Use and prepare equipment and supplies safely and effectively, consistently adhering to aseptic technique. Communication & Coordination Prepare clinical notes in accordance with professional licensing body and internal policy using the company’s electronic medical record and submit documentation timely. Attend and contribute to in‑services, case conferences, and other meetings as required by agency policy to ensure coordinated and comprehensive plans of care for the patients of the agency. Interact with team members in a positive and professional manner. Communicate plan of care to referring physician and other agency professionals. Maintain contact/communication with other personnel involved in the patient’s care to promote coordinated, efficient care. Additional Responsibilities Participate in personal and professional growth and development. Perform other duties as assigned by the registered nurse. Qualifications Graduate of an accredited Licensed Practical Nurse or Licensed Vocational Nurse program. Three (3) years nursing experience preferred; community, home health or medical/surgical experience preferred. Currently licensed as a licensed practical nurse or licensed vocational nurse in the state. Possess and maintain valid CPR certification. Comply with accepted professional standards and practice. Demonstrate good verbal and written communication and organizational skills. Licensed driver with reliable transportation.
