Community Care Navigator: Connect Services & Support

Neighborhood Health Plan of Rhode Island

Neighborhood Health Plan of Rhode Island seeks a Community Care Coordinator to manage care for members with social needs. Responsibilities include conducting outreach, screening health needs, and coordinating services to ensure patient-centered care. Ideal candidates will have 3-5 years of experience in a medical or social services setting and strong communication and organizational skills.
