About the Position

Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience License: Must have a valid nursing license in hand Certifications: CPR, BLS Must-Have: Proficient in HomeCare HomeBase and OASIS Description: Provides, coordinates, and directs the provision of home nursing care according to physician's orders based on agency policies and procedures. Utilizes the nursing process to apply competent care practices. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Conducts home visits and attends periodic office meetings. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Average number of patients per day is 5 to 8. Each visit and activity, including documentation and travel, holds a different point value. Assigned contractors will see between 5 to 10 patients per day, equating to roughly 6 to 8 points. No guaranteed hours for contractors; may discuss alternative assignments if census is low. Types of cases include Start of Care, Wound Care, and Follow-up Visits. First-timers are not accepted; recent specialty experience is required. Weekend requirements vary per location and profession. On-call requirements may vary; to be discussed during the interview. Must be willing to travel up to 75 miles from the home clinic. Mileage is reimbursed at the IRS standard rate. Local candidates are accepted. Pending licenses are not accepted; the license must be current. Home health experience is required. Must understand that documentation must be completed within 24 hours of each appointment. Delayed or incomplete documentation may result in cancellation. Must be open to on-call rotations. No guaranteed hours exist with this contract; weekly productivity points of 30 are required.
