Job Title Physical Therapist Location West Palm Beach, FL | Skilled Nursing Facility Schedule / Job Type Contract | Days | 13-Week Assignment Key Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans within a skilled nursing facility setting
- Deliver hands-on therapeutic interventions to improve patient mobility, strength, and functional independence
- Monitor and document patient progress, adjusting treatment plans as clinically appropriate
- Collaborate with interdisciplinary care teams to ensure coordinated and comprehensive patient care
- Educate patients and family members on home exercise programs and safe functional techniques
- Active Physical Therapist (PT) license in the state of Florida
- Graduate of an accredited Physical Therapy program
- Current BLS/CPR certification
- Prior experience working in a skilled nursing facility or similar post-acute care environment
- Experience with MDS assessments and skilled documentation in a SNF setting
- Familiarity with electronic medical records (EMR) systems
- Strong communication and patient-centered care skills
