Per Diem RN- Float Team LifePath Hospice

Job Summary The Per Diem RN is responsible for assessing and identifying patient/family needs, utilizing the nursing process, coordinating the Plan of Care with the Interdisciplinary Group (IDG), and providing palliative and supportive care to the patient/family unit. Key Responsibilities Obtains physician orders as needed. Provides and manages direct care to patients and families as part of IDG. Coordinates the Plan of Care, ensuring an individualized care plan reflects evolving patient needs. Educates patients, families, and staff about disease process, palliative interventions, and safety practices. Reports changes in patient condition to IDG or other health professionals. Participates with IDG to evaluate hospice admissions for level of care appropriateness. Attends patient deaths; prepares body for transport; oversees disposal of medications. Provides bereavement interventions to families and caregivers. Presents concise oral and written reports to IDG. Communicates accurately to physicians, staff, patients, families, and supervisors. Supervises patient care provided by LPNs, aides, and contract nurses. During emergencies, may report to designated locations to ensure continuity of services. Performs other duties as assigned. Qualifications Current RN license in the state where you will be working. Active BLS for healthcare professionals from the American Heart Association or Red Cross. Minimum one year nursing experience; hospice or hospital experience preferred. Experience with an EMR/EHR system. Valid driver’s license and automobile insurance; reliable transportation to meet visit schedule. Ability to use equipment with visual and auditory mechanisms. Effective communication in English (verbal and written). Flexible schedule to meet Hospice House patient needs. Ability to manage emotional stress of caring for terminally ill patients and families. Physical ability to access patient rooms, home, or other service areas, navigate stairs, narrow spaces, and tolerate heat and lack of air conditioning. Physical ability to perform essential functions (lifting, bending, pushing, kneeling) with or without accommodation. Employees at PACE must complete Alzheimer’s Disease and Dementia training through the Florida Department of Elder Affairs. Additional Required Training Cross training to hospice house or field positions when census requires flexible staffing. Complete clinical admissions for all services, including informed consents, initial assessments, and documentation per policy. Provide information and initiate Plan of Care for physicians. Perform skilled nursing interventions to prevent complications and alleviate symptoms. Provide care to GIP patients in hospital, including documentation and efficient discharge. Act as company representative at assigned facilities; facilitate referrals. Maintain clinical communication via SBAR and discuss needs with after-hours staff. Obtain consents, arrange safe discharge, and secure medical record information for referrals. Develop relationships with case managers, physicians, and other facilitators. Collaborate with Professional Relations Representatives to educate on available services. Competencies Complete competency requirements for this position. May also work in emergency conditions requiring additional duties.
