Description Whitney Young Health (WYH), in partnership with Capital Region Health Connections, is committed to providing comprehensive community-based case management services to adults with chronic medical conditions and co‑occurring mental health and substance abuse disorders, through Health Home Care Coordination across Albany and Rensselaer counties. Job Summary Using a patient-centered approach, Health Home Care Coordinators assist individuals enrolled in the program to manage their conditions and achieve optimal health outcomes by developing individualized care plans designed to link individuals to necessary services; such as housing, health care services, mental health and substance use providers, social services (food, entitlement benefits, transportation), community programs, and more. The ideal candidate possesses excellent written and verbal communication skills, knowledge of community resources, and strong time management and organizational skills. The person in this position demonstrates computer proficiency, good judgement, establishes healthy boundaries, works independently, and adheres to strict confidentiality guidelines. Requirements Minimum Qualifications Associate’s degree in human services or a related field, and previous experience working with people with behavioral health disorders, HIV/AIDS, children, families, ID/DD, or substance use disorders Valid NYS Driver License in good standing (verified annually), and proof of auto insurance in compliance with NYS mandatory limits and coverage Preferred Qualifications Bachelor’s degree in a health or human services related field such as psychology, sociology, social work, or nursing, with at least one year of related Health Home Care Coordination work experience in a medical setting.
