Schedule:

• Full-time hours, Monday through Friday
• On-call responsibilities may be required
• Structured day with established patient visit expectations

Qualifications:

• Active RN license in good standing
• At least 1 year of recent experience in home health nursing
• OASIS documentation proficiency required
• Familiarity with HomeCare HomeBase (strongly preferred)
• Reliable transportation and a valid driver's license
• Excellent organizational skills and flexibility to manage a full visit schedule

Responsibilities:

• Deliver skilled in-home nursing care and clinical interventions
• Conduct thorough patient assessments and complete OASIS documentation with precision and timeliness
• Coordinate care with physicians and the wider clinical team
• Provide wound care, medication management, disease monitoring, and patient/family education
• Report patient status updates promptly and ensure ongoing, high-quality care
