Care Team Coordinator

The Care Team Coordinator serves as an administrative and regulatory coordination resource for the interdisciplinary team (IDT), ensuring compliance with PACE regulations and organizational standards. This role coordinates Service Determination Requests (SDRs), Plan of Care (POC) meetings, documentation, and required assessments while supporting efficient communication among the interdisciplinary team. The IDT Coordinator promotes regulatory compliance, quality improvement, and timely care planning to ensure exceptional participant outcomes.

What is PACE?

PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors' health so they can live safely in their homes. There are 180 PACE programs, serving more than 80,700 participants in 33 states.

The number of people aged 55+ is increasing in Grand Junction, CO and the demand for eldercare professionals is on the rise.

What You Bring:

• High school diploma or equivalent required, associate degree is preferred.
• Minimum of 3 years’ experience in a healthcare administration role preferred.
• Experience with medical scribing or medical transcription preferred.
• Minimum of 1 year of experience working (paid or volunteer) with the elderly.
• Experience in a PACE program is highly desirable.
• Strong organizational and time management skills.
• Excellent written and verbal communications skills.
• Skilled in meeting coordination
• High attention to detail.
• Ability to manage multiple priorities simultaneously.
• Proficiency with Microsoft Office and electronic medical records.
• Ability to build collaborative working relationships.
• Commitment to customer service and participant-centered care.
• Ability to interpret written procedures, regulations, and operational guidelines.

• Competitive pay structure starting at $22.60-$25.00 per hour, depending on experience.
