Job Description:


This opportunity is based in Montrose, CO, serving patients through HopeWest Hospice. This role offers the chance to practice truly patient-centered care while making a meaningful difference in the lives of patients and their families.


As a Hospice RN, you will provide compassionate, holistic care and serve as both a clinician and educator-guiding patients and caregivers through care plans, symptom management, and end-of-life support.


Primary RNs manage a dedicated caseload and lead the interdisciplinary team, ensuring personalized care wherever the patient calls home, including:

• Private residences
• Nursing homes
• Assisted living facilities
• Group homes

Shift Information:

• Monday to Friday (8:00 AM - 5:00 PM)
• Occasional weekend backup and on-call responsibilities

Responsibilities:

• Manage assigned patient caseload
• Lead and coordinate interdisciplinary care teams
• Provide symptom management and clinical assessments
• Educate patients and caregivers on care plans and goals
• Deliver care across multiple home-based settings

Requirements:

• Active RN license
• Strong clinical and assessment skills
• Experience in hospice or home health preferred
• Ability to work independently and manage caseload effectively
