We are looking for a passionate and driven Community Outreach Coordinator to join our team in Glendale, AZ. In this role, you'll be at the heart of connecting patients with the programs and services they need, making a real difference in people's lives every day. If you thrive on building relationships and making a community impact, this is the opportunity for you. Responsibilities:
- Develop and execute community outreach strategies to identify and recruit eligible patients
- Build and maintain relationships with community organizations, healthcare providers, and local partners
- Conduct outreach activities at community events, health fairs, and partner locations
- Screen and enroll prospective patients into programs, ensuring eligibility requirements are met
- Maintain accurate records of outreach activities, patient contacts, and recruitment metrics
- Serve as a trusted resource and point of contact for patients navigating available services
- Collaborate with internal teams to align outreach efforts with organizational goals
- Experience in community outreach, patient recruitment, or a related healthcare/social services role
- Strong interpersonal and communication skills with the ability to connect with diverse populations
- Knowledge of local Glendale/Phoenix Metro community resources and healthcare landscape is a plus
- Ability to work independently, manage time effectively, and meet recruitment goals
- Proficiency in maintaining accurate documentation and data entry
- Bilingual (English/Spanish) preferred but not required
- Valid driver's license and reliable transportation for community-based activities
