As part of our team, you'll collaborate closely with nursing professionals, physical therapy assistants, and other members of the interdisciplinary team, all dedicated to ensuring holistic care delivery.Key ResponsibilitiesIn your role as a Home Care Physical Therapist, you will:Develop and implement personalized physical therapy treatment plans that align with each patient's unique needs and physician's prescriptions.Oversee and guide physical therapy assistants to ensure the effective execution of care plans in home environments.Evaluate clients' functional abilities through diagnostic tests to inform treatment decisions, identifying areas for improvement.Adapt therapeutic exercises and functional activities tailored to the patient's home setting to promote independence and safety.Perform assessments of the home environment to recommend modifications that will facilitate rehabilitation.Maintain detailed clinical records, including evaluations and progress notes, and collaborate on updates to care plans as necessary.Work diligently to advocate for the needs of clients and their families, ensuring clarity in communication and engagement throughout the treatment process.Be an active participant in interdisciplinary team meetings, contributing insights that enhance the overall care strategy.Supervise and mentor Physical Therapy Assistants in executing specific aspects of patient care.Ensure adherence to all federal, state, and local regulations concerning patient care and documentation.Patient Population / Client FocusOur services cater to a diverse client base, including patients recovering from surgeries, managing chronic conditions, and those seeking palliative care. Each client presents unique challenges; thus, you will tailor interventions to promote functional capacity, enhance quality of life, and support independence in daily living.QualificationsTo succeed in this role, ideal candidates will possess:A Bachelor's degree in Physical Therapy from an accredited institution; a Doctorate in Physical Therapy is preferred.A valid physical therapy license in good standing within Virginia.Current CPR certification.A valid driver's license and automobile insurance for travel to client homes.Exceptional communication and interpersonal skills to foster strong relationships with clients and their families.A proactive, collaborative approach to patient care within a team environment.A creative mindset with the ability to implement innovative therapies tailored to individual needs.Preferred ExperienceWhile we require a minimum of one year of experience in physical therapy, preference will be given to candidates with experience in acute care or home care settings. Familiarity with electronic health records and regulatory compliance in home healthcare is highly desirable.Compensation and BenefitsHeal & Revive Chiro Clinic offers a competitive annual salary ranging from USD 90,000 to 100,000, commensurate with experience.
