About the Position

Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum of 2 years of Acute Care Case Management/Discharge Planning experience is required. Client experience is mandatory. License: A valid state nursing license is required. Pending licenses may be accepted but must be active upon start date. Certifications: No specific certifications are required. Must-Have: Behavioral health experience does not apply. Must have experience with assessing patients, care coordination, discharge planning, and utilization review. Ability to organize and participate in family meetings is essential. Knowledge of treatment options for substance use disorders, mental health history, and patients experiencing homelessness is important. Competent with arranging durable medical equipment and transport services independently. Description: This role involves managing patient care and ensuring effective discharge planning. Responsibilities include coordinating care for patients and their families. You will participate in developing treatment plans in collaboration with interdisciplinary teams. Experience with various health insurance plans and benefits eligibility will be necessary. This position requires working every other weekend to meet departmental needs. You may be expected to float between different facilities as required. Duties may include arranging skilled nursing facility placements and other community resources. Documentation and compliance with various regulations and standards are required.
