Job Details: Location: Exposition Park - Los Angeles, CA 90007; Position Type: Part-Time; Education Level: B.A./B.S.; Salary Range: $23.40 - $28.02 hourly; Job Category: Nonprofit - Social Services. ROLE SUMMARY Case Managers provide an array of services and activities in support of access to medical, educational, social, prevocational, rehabilitative, or other needed community services for eligible clients and families. The Case Manager conducts groups for children, high-risk teens, parents of young children and adults, focusing on building healthy relationships and promoting positive parenting/relationships, using a strengths-based approach to empower families and support a community-based philosophy aligned with Wellnest’s mission, vision, and core values. ESSENTIAL RESPONSIBILITIES Conduct comprehensive needs assessments with clients and assist them in obtaining needed resources such as health insurance, housing, and low-cost child care through referral and linkage. Provide individual and group rehabilitation services under Department of Mental Health contract or other agency funding. Meet agency’s productivity standards for client service hours. Complete and submit progress notes in a 24-hour timeframe using the agency’s electronic health record system and collaborative documentation while serving clients; discuss and receive proof of insurance/Medi-Cal information in the field and submit to TQM and/or IS Support Team. Provide guidance and assistance to children, parents/caretakers, and adults on managing behaviors successfully. Provide guidance and assistance to children, parents/caretakers, and adults on managing familial stressors effectively and successfully. Assist clients with identifying internal resources to assist with multiple stressors such as extended family members, grandparents. Develop and provide client rehabilitation services which assist in improving, maintaining, or restoring daily functioning at home, school, and community in accordance with treatment goals and in collaboration with treatment team (i.e., therapist, psychiatrist). Provide interagency and intra-agency consultation, communication, coordination and referrals. Identify and communicate to therapist obstacles preventing clients from connecting with program and/or referral services. Take initiative to represent Wellnest while supporting the mission by engaging actively in community outreach events, including events related to recruitment of clients and funder activities and, on weekends. Attend community meetings as requested by program director. Represent the agency at community resource fairs. Follow up with families that drop out of treatment. Provide services in the communities (in client’s homes, school or other field-based settings). Conduct presentations in the community to help reduce stigma of mental health. REQUIRED QUALIFICATIONS Bachelor’s degree in social work, psychology, or other mental health related discipline. Must speak Spanish. Must possess and maintain a valid CA driver’s license and insurable driving record that meets Wellnest’s insurance carrier’s criteria for liability coverage. Use of a personal vehicle is required to complete the essential duties of this position. Must complete a test for Tuberculosis (TB), and fingerprint clearance by the Department of Justice. CONDITIONS OF EMPLOYMENT Employee may be asked to pursue additional training when it is determined to be in the best interest of the Company. WORKING CONDITIONS Employee regularly works out in the field and in an office environment. Employee may be required to work flexible, evening and/or weekend hours for time-sensitive projects. REASONING ABILITY Ability to effectively present information and respond to questions. Ability to read, analyze and interpret manual and office documents. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form. Ability to define problems, gather, transcribe and post data, analyze, synthesize and classify information, and draw valid conclusions. Ability to work independently and with others - including other employees, clients and members of the public - in face-to-face and telephonic contexts. Ability to multi-task, establish priorities and manage time, meet deadlines and perform duties under time constraints. Ability to be flexible and adapt to changing work demands. Maintain a high level of concentration and attention to detail for extended periods of time. Maintain a high level of ethical and professional standards in accordance with agency policy. PHYSICAL DEMANDS Ability to talk or hear in order to give and receive information and instructions. Ability to stand, walk, sit, use hands to finger, handle or feel objects, tools or controls. Ability to reach with hands and arms. Ability to use computer keyboard up to 50% of the day. Visual acuity, ability to adjust focus and peripheral vision for work on a laptop or other computer equipment. Lift and/or move up to 25 pounds. Able to use telephone, office copier, calculator, fax machine, computer printer and scanner, or other equipment as required.
