In this essential role, you will facilitate the admissions process, ensuring smooth transitions for clients and families while maintaining accurate documentation and coordinating with our care teams. **Key Responsibilities**

• Conduct initial client assessments and intake interviews to determine care needs and service eligibility
• Process admissions paperwork, insurance verifications, and coordinate with healthcare providers to ensure seamless service initiation
• Maintain accurate client records and documentation in compliance with healthcare regulations and company policies
• Serve as primary liaison between clients, families, and internal care teams to address questions and resolve concerns
• Schedule and coordinate initial care visits, matching clients with appropriate caregivers based on needs and preferences
• Monitor admissions workflow and follow up on pending applications to ensure timely service delivery

**Requirements**

• Bachelor's degree preferred or equivalent experience, with 1+ years of experience in healthcare, customer service, or related field
• Healthcare background with understanding of home care services, insurance processes, and medical terminology
• Proficiency in Microsoft Office Suite, data entry, and electronic record keeping systems
• Excellent communication and interpersonal skills with ability to handle sensitive situations with empathy and professionalism
• Strong attention to detail, problem-solving abilities, and time management skills with capacity to multitask in fast-paced environment
