The RN Lead is empowered to practice and contribute throughout the care experience. At its core, the RN Lead provides direct patient care while demonstrating servant leadership, leading clinic-wide huddles and leveraging our interdisciplinary approach to integrated care. The RN Lead promotes a level of care that goes beyond industry standards and strives for excellence at every step. In addition, the RN Lead is responsible for directing and supervising a team of nurses.Performs evidence-based nursing care and patient educationPerforms care management by connecting patients with specialists and following up to ensure patients are getting the care they needUtilizes population management tools to ensure patients receive optimal care based on industry-wide recommendationsProvides phlebotomy services, performs in-house laboratory tests and works with the provider to determine how and when to send out specialty labsScreens for vaccine-preventable diseases and administers primary care and travel-related immunizationsManages RN schedule and appropriateness of appointments. Takes initiative to contact patients when needed and demonstrate knowledge of when to consult with a providerComplies with all policies, procedures, and protocols of the practice, including performing specific care under standing ordersProvides general triage services to walk-ins and over the phone. xhyhwjd Assesses a patient's needs, consults with providers when needed and directs appropriate care.Provides occasional on-site care at client-sponsored events such as flu clinics and biometric screeningsParticipates in quality improvement programs to ensure the practice is performing efficiently and in accordance with Crossover's policiesDemonstrates proficiency in the use of healthcare analytic tools, systems, and analysisParticipates in implementation of new health technologies, products, services, and programs that establish the Crossover Medical Group as a recognized thought leader for employer-based health care and lifestyle medicine programsManages population health programs and clinic outreach initiativesServes as a resource to other nurses and the care team, and participate in the training and on-boading of new nursesAssists in performing Quality and Compliance checks required on a daily, weekly and monthly basisContributes to reporting requirements related to incident reporting and compliancePerforms other duties as assignedRequired Qualifications Graduate from an accredited school of nursing5 years of clinical experienceCurrent Registered Nurse license in the state you are practicingCurrent BLS (Basic Life Support) certificationPhlebotomyPreferred QualificationsBachelor of Science Degree in Nursing (BSN)Outpatient care and urgent care experience
