Speech Language Pathologist, (SLP) - Home Health

Job Summary BAYADA Home Health Care seeks an experienced Speech Language Pathologist (SLP) for a per diem opportunity performing home health visits in Greensboro, North Carolina. The role serves adult and geriatric patients in the Rockingham County area. Responsibilities Make home visits to clients in designated geographic territories. Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for patients with speech, language, communication, or swallowing difficulties. Develop and revise treatment plans based on evaluations, physician orders, and goals. Participate in multidisciplinary care planning with BAYADA clinicians. Educate and instruct patients, families, or representatives regarding rehabilitative care and activities that promote health, safety, and independent living. Accurately document observations, interventions, and evaluations in the electronic medical record using a touch‑pad tablet. Qualifications Current licensed Speech Language Pathologist in North Carolina. Certificate of clinical competence from the American Speech‑Language‑Hearing Association or equivalent educational and work experience. Must have home health experience and be available 2–3 days per week. Solid computer skills; experience with electronic medical records is preferred.
