Role Description This is a part-time hybrid role for a Nurse Practitioner based in Greater Orlando Area FL, with opportunities to work both on-site and remotely. The Nurse Practitioner will provide patient-centered primary care and chronic disease management for individuals in assisted living facilities, group homes, and private residences. Responsibilities include performing health assessments, developing care plans, conducting follow-ups, managing transitional care, and collaborating with other healthcare providers and community partners to ensure comprehensive care. This role also involves using telehealth technology for virtual consultations and coordination of services. Qualifications Clinical skills in primary care, chronic care management, and transitional care services Experience with telehealth consultations and care coordination Strong communication and interpersonal skills for patient and team interactions Familiarity with healthcare technology and electronic health record systems Ability to work both independently and collaboratively in a hybrid work environment Current licensure as a Nurse Practitioner in the state of Florida Certification in adult or family practice preferred Prior experience in home healthcare, ALF, or group home settings is a plus #J-18808-Ljbffr Olive Health
