Location: Hialeah, FL (Miani-Dade County)

*Field-Based Role – 75% Travel Required*

Shift: Monday–Friday, 8a-5p

Compensation: $34.13/hour + mileage reimbursement

Overview:

We are seeking a bilingual Spanish/English Case Management Coordinator to support members throughout the West Miami and Doral area. This is a field-based healthcare coordination role focused on helping members access medical care, community resources, and long-term support services. This role will require 50-75% travel for face-to-face visits with members in the Hialeah ( Must be in 33010, 33012, 33013, 33014, 33015, 33016, 33018) area of Miami-Dade County, FL.

This opportunity is ideal for candidates who enjoy working independently, building relationships in the community, and coordinating care for members with complex healthcare needs.

Responsibilities:

• Coordinate healthcare services and support for Medicaid Long Term Care members.
• Assess members’ medical and social needs to help create care plans.
• Work with doctors, caregivers, and healthcare teams to ensure proper care.
• Monitor member progress and update care plans as needed.
• Help members access healthcare services, resources, and benefits.
• Educate and motivate members to make healthy lifestyle choices and follow treatment plans.
• Document case activities and maintain accurate records while following healthcare guidelines and policies.

Qualifications:

• Fluent bilingual Spanish/English required (speaking, reading, and writing)
• Education: Bachelor’s degree in social work or related field required.
• Must reside in one of the following zip codes: 33010, 33012, 33013, 33014, 33015, 33016, 33018
• Previous case management, long-term care, healthcare coordination, social services, and/or managed care experience preferred

Additional Information:

• Will require 50-75% travel for face-to-face visits with members in the Hialeah
• Remote training provided via Microsoft Teams for 4–6 weeks
• Monday–Friday schedule with flexibility and autonomy in the field
