In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Boston MA

Position Overview Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations engaging with people in the comfort of their own homes and bridging gaps in care. Job Highlights The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, if ordered by the person's health plan, additional diagnostic tests (e.g., diabetic eye exams, spirometry). You will use an iPad and an intuitive clinical workflow for each evaluation. Prescription, lab ordering or alterations to current treatment regimens are not required. Work to visit millions of people nationwide and connect them to the right care for their unique needs. Collaborate with Signify Health as a clinician colleague, fulfilling the specified role requirements. Be flexible to travel locally and within licensed states as required. Conduct virtual visits as needed. Provide all Signify Health services, including Diagnostic & Preventive Services products such as spirometry, DEE, bone density screening, etc. Work Environment Note Your primary workplace is the private residence of members. You must be able to perform duties safely in homes that may contain household pets, secondhand smoke, or other airborne allergens or irritants. Qualifications (NPs and PAs) Active, unrestricted license(s) in coverage area(s). Board certification (required). Multi-state licenses (preferred). Ability to cover Boston and surrounding area. Salary Anticipated Weekly Hours: 40 Time Type: Full time Pay Range: $95,738.00 - $206,206.00 (base hourly rate or base annual full-time salary; actual depends on experience, education, geography, and other relevant factors).
