Physician Assistant

Signify Health is seeking a mission-driven Physician Assistant to perform comprehensive in-home and virtual health evaluations for diverse, often vulnerable patients. You will conduct detailed histories and physicals, manage chronic conditions, reconcile medications, and document findings in our EHR to support value-based care programs. Partnering with interdisciplinary teams, you’ll use data and analytics to identify care gaps, coordinate follow-up, and educate patients on managing their health.
