Community RN Care Manager for Dual-Eligible Adults

CareSource is seeking a Clinical Care Manager in Massachusetts to provide clinical care management for dually-eligible enrollees. This registered nurse will engage with enrollees in their homes and coordinate care with healthcare providers. The ideal candidate will have a nursing degree, a strong background in care coordination, and the ability to support individuals with complex medical and social needs. This position requires excellent communication and problem-solving skills. A valid driver's license and experience in Medicaid/Medicare are preferred. #J-18808-Ljbffr CareSource
